Next Gen NCLEX, decoded: case studies, item types and partial credit

Every NCLEX-RN has 3 NGN case studies (18 items). The 6 clinical judgment steps, the question types and NCSBN's 3 partial-credit scoring rules, explained.

The short answer

The Next Generation NCLEX (NGN) launched on April 1, 2023 to measure clinical judgment (NCSBN). Under the 2026 RN Test Plan, every NCLEX-RN includes three case studies of six items each (18 items) plus about 10% stand-alone clinical judgment items (NCSBN 2026 RN Test Plan).

  • Each case study walks through the six steps of NCSBN's Clinical Judgment Measurement Model, from recognizing cues to evaluating outcomes.
  • Items with more than one correct answer earn partial credit under three rules: plus/minus, zero/one and rationale scoring.
  • Under plus/minus scoring, wrong picks cost points, so over-selecting hurts.

The Next Generation NCLEX (NGN) didn't just add new question formats. It changed what the exam is trying to measure: not whether you can recall a fact, but whether you can think through a changing patient situation like a new nurse would.

The model behind it: six clinical judgment steps

NGN questions are built on the NCSBN Clinical Judgment Measurement Model (NCJMM). The 2026 RN Test Plan describes a case study as six items about the same client, with information that unfolds as you go, and one item for each of the model's six steps:

  1. Recognize cues: which findings matter? (history, vital signs, labs)
  2. Analyze cues: how do they connect to the client's presentation?
  3. Prioritize hypotheses: what's most likely, most urgent or most risky?
  4. Generate solutions: what are the expected outcomes, and which interventions get there?
  5. Take action: carry out the solution for the highest priority.
  6. Evaluate outcomes: compare what happened with what you expected.

How much of the exam is NGN?

  • Three case studies, 18 items, on every NCLEX-RN, whatever the exam length.
  • About 10% stand-alone clinical judgment items, chosen depending on how long your exam runs.
  • The case studies span any content area, so they're counted separately from the Client Needs percentages.
  • Of the minimum 85 items, 52 are scored Client Needs items, 18 are case study items, and 15 are unscored pretest items you can't tell apart. See How the NCLEX actually works.

Formats you'll see

  • Matrix / grid: rows of findings or actions, columns of options.
  • Bowtie: a condition in the middle, actions on one side, parameters to monitor on the other.
  • Highlight: click the parts of a chart or nurse's note that matter.
  • Drop-down: complete a sentence by picking the right word.
  • Drag-and-drop: order steps or sort options.
  • Extended multiple response: the new form of select-all-that-apply.

Partial credit: the three scoring rules

NCSBN gives partial credit on any item with more than one correct answer, using one of three rules (NCLEX FAQs):

  • Plus/minus (+/–): used when you can select an unspecified number of options, like select-all-that-apply. You gain a point for each correct choice and lose one for each incorrect choice. The minimum score is zero.
  • Zero/one (0/1): used when you're told to choose a specific number of responses, such as "select 2" or one choice per row. You earn a point for each correct response, and an incorrect one simply earns nothing.
  • Rationale: used when you evaluate closely linked information, such as a condition and its cause. Both parts of the pair must be right to earn the point.
Try it

A select-all-that-apply item has 3 correct options. You select 3 correct options and 1 incorrect option. Under +/– scoring, what do you earn?

Frequently asked questions

How many NGN case studies are on the NCLEX-RN?

Three, with six items each, for 18 case study items on every exam. There are also about 10% stand-alone clinical judgment items, depending on exam length (NCSBN 2026 RN Test Plan).

When did the Next Gen NCLEX start?

The NGN launched on April 1, 2023, according to NCSBN.

Do you get partial credit on the NCLEX?

Yes, on items with more than one correct answer. NCSBN uses plus/minus, zero/one or rationale scoring depending on the item. Standard single-answer multiple choice is still right or wrong.

Can you lose points for wrong answers on the NCLEX?

Only on plus/minus items, where each incorrect selection subtracts a point. The item can't score below zero, so it never pulls down your other items.

What are the six steps of the Clinical Judgment Measurement Model?

Recognize cues, analyze cues, prioritize hypotheses, generate solutions, take action and evaluate outcomes. Each NGN case study has one item for each step.

Is the Next Gen NCLEX harder?

It asks you to apply judgment to an unfolding client situation rather than recall facts in isolation, which many students find harder at first. Partial credit means a mostly right answer still earns points, though.
